Appointments

Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.
No one wants to suffer through a broken tooth or toothache. We want to be there when you need us. Our team provides same day appointments for emergencies in most cases. If an emergency occurs after hours, contact our office and we will do our best to serve your needs.
A $50 nonrefundable fee may be assessed for appointments cancelled or rescheduled without 24 hour notice.

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.

Payment

We accept the following forms of payment: Cash, Check, Visa and MasterCard. A 3% convenience fee will apply to all credit card transactions over $100 Payment plans are available for those who need them. Please notify us if this is an option you need to consider.
50% payment for services is due at the time services are scheduled unless prior arrangements have been made with the doctor and the billing receptionist. The remaining balance is due the day of treatment.
The parent that accompanies the minor child/children to the appointment is responsible for any payment due. For unaccompanied minors, non-emergency treatment will be denied unless charges have been pre-authorized or previous arrangements have been made with the doctor and billing receptionist.
